Understanding the Dengue Virus Timeline
The dengue illness follows three distinct phases — and the correct test depends entirely on which phase the patient is in when they present.
Dengue fever is caused by one of four dengue virus serotypes (DENV-1 to DENV-4), transmitted by the Aedes aegypti mosquito. Understanding the viral and immune timeline explains why different tests are needed at different days of illness — and why ordering the wrong test produces false negatives.
Viraemic Phase
The dengue virus multiplies rapidly in the bloodstream. The NS1 viral protein is detectable in blood. The immune system has not yet produced antibodies.

This is why NS1 is the only reliable dengue test in the first 3–4 days of fever, before the immune system has had time to produce detectable antibody levels.

Dengue IgM & IgG Antibody Tests
IgM (Immunoglobulin M) is the first class of antibody produced by the immune system in response to a new dengue infection. IgM antibodies typically become detectable in blood from Day 4–5 of fever and remain elevated for 3–6 months after infection ends.
Ordering dengue IgM in the first 3 days of fever will almost always return a false negative — the antibody simply has not developed yet. This is the most common and dangerous diagnostic mistake made in dengue management across India.

Detectable from Day 4–5. Peaks in the second week. Remains elevated 3–6 months. Confirms acute or recent dengue infection. Most sensitive and specific test from Day 5 onwards.
🔵 Dengue IgG — Secondary Infection Marker
Appears from Day 7–14 and persists for years. A positive IgG alongside IgM indicates secondary dengue — a prior infection with a different serotype. Secondary dengue carries significantly higher risk of haemorrhagic complications.

Dengue can cause platelet counts to crash from normal levels to dangerously low within 24–48 hours. The platelet count determines dengue severity and guides hospitalisation decisions.
🔬 Haematocrit — Plasma Leakage Marker
Haematocrit (PCV) is the percentage of red cells in blood. In dengue, plasma leaks out of blood vessels, concentrating the red cells — causing haematocrit to rise.
A rising haematocrit above 20% from baseline is a warning sign of severe dengue with plasma leakage. This requires urgent IV fluid management and hospital admission.

Warning Signs — When to Go to Hospital Immediately
These signs indicate progression to severe dengue. Do NOT wait for the next scheduled test. Go to emergency immediately.
Persistent vomiting — more than 3 episodes in 24 hours
Severe abdominal pain or tenderness that worsens rapidly
Bleeding from gums, nose, under the skin (petechiae or bruising)
Blood in vomit, urine or stool — any amount
Confusion, restlessness or any change in consciousness
Cold, clammy, pale skin — sign of dengue shock syndrome
Emergency: If a patient with dengue fever develops any of the above warning signs, do NOT wait for the next scheduled blood test. Proceed immediately to the nearest emergency department. Dengue shock syndrome is life-threatening but highly treatable when caught early.
